For individuals who are overweight or obese in whom pharmacologic therapy is indicated, we suggest subcutaneous tirzepatide or semaglutide. The selection between tirzepatide and semaglutide depends on patient preferences and comorbidities.

Candidates for drug therapy include individuals with a BMI ≥30 kg/m2, or a BMI of 27 to 29.9 kg/m2 with comorbidities, who have not met weight loss goals (weight loss of at least 5 percent of total body weight at three to six months or unable to maintain this weight loss long term) with a comprehensive lifestyle intervention.

Subcutaneous Semaglutide has demonstrated efficacy for weight loss in trials involving patients with and without type 2 diabetes. It can also improve glycemia and lipids and has demonstrated cardiovascular benefits.

What is Semaglutide/ Cyanocobalamin (Vitamin B12)?

Semaglutide is an injectable medication that works by helping the pancreas to release the right amount of insulin when blood sugar levels are high. Insulin helps move sugar from the blood into other body tissues where it is used for energy. Semaglutide injection also works by slowing the movement of food through the stomach and may decrease appetite and can help achieve weight loss goals!

How can Semaglutide be Used?  

Semaglutide can be used for Type 2 diabetes as well as weight loss. Semaglutide interacts with the parts of the brain that suppress your appetite and signal you to feel full. When used in conjunction with diet and exercise, it can cause significant weight loss — and a reduced risk of cancer, diabetes and heart disease — in people who are obese or overweight. Semaglutide also prompts the body to produce more insulin, which reduces blood sugar (glucose).

You should not use semaglutide if you are allergic to it, or if you have: 

  • multiple endocrine neoplasia type 2 (tumors in your glands); or
  • a personal or family history of medullary thyroid carcinoma (a type of thyroid cancer).

  • Pregnant or Breastfeeding 
  • Diabetes Mellitus Type 1  
  • Volume Depletion

Tell your doctor if you have ever had: 

  • a stomach or intestinal disorder;
  • pancreatitis;
  • kidney disease; or
  • eye problems caused by diabetes (retinopathy). 

What is Tirzepatide?

Tirzepatide is a diabetes medication, marketed under the brand name Mounjaro. The US Food and Drug Administration recently approved subcutaneous tirzepatide for chronic weight management. Two randomized trials in adults with obesity demonstrated mean losses of 15 to 21 percent body weight with the highest dose of tirzepatide (15 mg weekly). In the larger of the two trials, over 80 percent of participants in all tirzepatide treatment groups (5 to 15 mg weekly) lost ≥5 percent of body weight, compared with 35 percent of those assigned to placebo.

Side Effects

Dose-related gastrointestinal side effects (nausea, diarrhea, constipation) were common but generally mild. Serious but less common side effects include pancreatitis, gallbladder disease, and kidney impairment.
Hormonal Contraceptives: Weight Loss injections may decrease the serum
concentration of Hormonal Contraceptives.
